PHARMACOLOGICAL ACTION
Niacin (nicotinic acid) has been used as an effective cholesterol lowering agent for many years by some clinicians. Its more general use has been hampered by the fact that doses in excess of 100mg may cause unpleasant flushing of neck and face and that the high doses (up to 2500mg) required by some patients may cause liver damage. The benefits of niacin without the side effects can now be obtained by using inositol hexanicotinate (IHN), an ester form of niacin which slowly releases niacin. The main benefits of niacin is that it effectively lowers total cholesterol and LDL cholesterol as well as triglycerides while at the same time raising HDL levels, thus establishing a favourable lipid profile in the heart patient (Am J Cardiol 1998, 82(12A): 74U-81U). IHN improves lipid patterns by reacting with lipoproteins in the blood and by decreasing VLDL’s in the liver (Alt Med Rev 1996,1 : 176). A readily available source of chromium is included to increase the effective dose. In some patients raised homocysteine have been observed after taking free niacin (Am Heart J 1999, 138 : 1082). As a safeguard against this effect, and with the additional benefit of lowering homocysteine levels, folic acid is also included.

INDICATIONS
Lipid abnormalities, especially raised total and LDL cholesterol and raised triglyceride levels.

CONTRA INDICATIONS
Severe liver conditions.

WARNINGS
Patients who are already on cholesterol lowering medication may require dose adjustments. This is best done under medical supervision. When the product is used over extended periods (more than a year), medical guidance may be necessary. Do not allow total cholesterol to drop to below 4mmol/l. Always consult your health professional if you are seriously ill.

INTERACTIONS
May enhance the effects of other cholesterol-lowering substances. Concomitant use with anti-coagulant substances may increase the risk of bleeding. Concomitant with anti-diabetic compounds may interfere with blood glucose control.

PREGNANCY AND LACTATION
Insufficient reliable evidence exists; avoid using.

DOSAGE AND DIRECTIONS OF USE
Take 3 capsules after breakfast. Although the dosage may be increased to 6 capsules per day this is seldom required. Optimum cholesterol levels are generally regarded as 5 to 5.2 mmol/l.
